Registration Information
When checking in for both inpatient and outpatient services you will be required to sign the conditions of admission form, which is a consent for treatment and authorization to bill insurance.
Remember to bring with you at the time of registration the following items:
- All active insurance cards
- Photo ID
- Living will, if available
- A list of current medications
- Any personal items that you may need during your stay
